I used to say “Gesundheit” (German parentage ya know) until a coworker insisted quite seriously that “God bless you” was the Only Proper Response. Now I just let folks sneeze without a wish for their good health.

Nope. If it concerns me I will ask if they are okay, or if they need anything (such as a tissue).
It does not occur to me that their souls are in trouble, and I am generally against triggers that I don’t choose for myself. It is tough, though. Sometimes when I am in a group of people and someone sneezes, everyone says it except me.
Lemmings. ^_^

I say Bless you, salut, bostrovnia, barook-hashem or ewww, depending on how close I am.
I also say excuse me when I fart, but I don’t really want anyone to make excuses for ne, nor am I asking pardon to leave the immediate area (unless I just ate some freakin’ carnitas).

Bless you. Or salud. I notice if others don’t say it.
